Younger and Slimmer in an Afternoon? Aesthetic Truths.
Ballito aesthetic surgeon Dr Sham Ally shares some of the latest procedures available which really can produce younger looking faces and slimmer bodies from simple, outpatient procedures.

Is it really possible to become younger and slimmer after one procedure?
Slimming with Liposuction:
“A surgeon has to work hand in hand with nature,” says Ally. “We work with what we have. and I am realistic with my patients with what they can ultimately expect. The perfect air-brushed image you see in magazines is not possible – for anyone. I counsel my clients to what they can really expect. Depending on the body shape, 4 cm reduction at the waist is realistic. Fat is not a heavy substance, so although we may remove a liter or so of fat, the scale will not drop that much – some patients quickly drop a dress size or two once the swelling goes down, others find clothing just fits better. Many of my clients find the procedure gives them the impetus to lose further weight and adapt a healthier lifestyle.”
Dr Ally has a strict satisfaction guarantee on her liposuction procedures and will do a re-do for free if results are not in line with her expectations.
Dr Ally has recently incorporated the Ultrasound Liposuction technique at her clinic. This latest improvement on the laser liposuction technique (laser probes melted fat pockets to enable the fat to be suctioned out using thinner, less invasive cannulae) has drastically reduced the time of procedure: a 4 1/2 hour procedure on hips and abdomen can now be done in 2 1/2 hours. The recovery results are also quicker and more effective. Most clients achieve 90% of their final result within 6 to 8 weeks of the surgery. The final reduction happens slowly over time as the body rids itself of broken fat cells naturally over the following months.

Consultation |
Dr Ally will conduct a thorough medical overview with blood tests to insure no underlying issues that may negate the effectiveness of the procedure. Insulin Resistance and Thyroid Problems are contra-indicated for this procedure. |
Day of procedure |
The patient is encouraged to enjoy a light, healthy meal before driving him/ herself to the clinic, wearing loose, comfortable clothes. The patient is dressed in a surgical gown and Dr Ally will use a marker to plan areas of fat removal. |
Procedure |
The doctor may administer a general painkiller such as Myprodol for the relaxation of the patient.A local anaesthetic is applied to the skin before a small (1 cm) incision is made in the belly button (belly fat) and each hip (for waist and hips) to allow insertion of a long, thin cannula under the skin.The doctor will distribute anaesthetic under the skin, into the areas where fat is to be removed. This is the most uncomfortable part of the procedure, particularly in the area above the ribs. The patient is awake and can advise the doctor on the efficacy of the anaesthetic. Dr Ally will adjust levels accordingly.Using a thin rod, a piezoelectric crystal converts electric energy to rapid (ultrasonic) vibrations and heat that are transmitted into the subcutaneous fat, destroying fat cells and breaking them into smaller molecules to be sucked out of the body.The doctor and her nursing assistant closely monitor tumescent fluid inserted into the fat cavity to mobilize and allow the aspiration of both fat and liquid.The patient is conscious at all times, can converse with doctor and nurse and may ask for a ‘comfort’ break at any time to visit the toilet.The nurse will feed the patient snacks for nourishment throughout the procedure. |
Final Stage of Procedure |
After all aspiration (sucking out) of fat is completed, the doctor and nurse will squeeze excess fluid from the incisions.A fountain of bloody fluid may be expelled from each incision. A simple dressing is placed over each cut and the client will be helped into his/her compression garment. The compression garment will be worn 24/7 for approximately 3 weeks post procedure to reduce swelling and oedema. Compression will then be required 12 hours a day for a further 3 weeks. |
Post-Op |
The client will dress him/ herself and enjoy a light meal with a sweet hot drink at the clinic to stabilize sugar levels after the procedure.When ready, he/she may drive him/herself home.High levels of subcutaneous fluid (a mixture of tumescent fluid and the body’s own anti-inflammatory response) cause oedemic swelling of enormous proportions. Loose clothing will be welcomed to accommodate the Tele Tubby effect of such swelling. |
At Home |
The effects of the subcutaneous anaesthetic will be wearing off as the patient returns home. General painkillers (Myprodol) taken as prescribed will ease the discomfort.It is recommended to take it easy on the first day home. By the second day, the patient should feel well enough to fulfill most tasks. |

“The Thread Facelift is minimally invasive. Using a biodegradable poly lactic acid thread used in cardiac, opthamology and plastic surgery for years, we make two tiny incisions in the face/ jawline. The suture has a number of small cones in the center which serve to catch or anchor the flesh: pulling either end of the suture threaded through the skin layer, we are able to lift the face to the desired level.” Explains Ally.
